Email is self-explanatory, Страна means country (for me it self-completed correctly, but google translate can probably help you check). Then the left form wants credit card number, date of expiry and full name, and right form wants credit card security number.

this is such a fucking important project to me because i am constantly stared at in public in a negative way and turned into some disgusting object for the amusement of others and this is a peaceful way to confront those people
